PivotPathways - Pivot, Inc.
Description
Pivot, Inc. relied on paper records and separate digital tools to support young people experiencing homelessness, transitioning out of foster care or involved with child welfare or juvenile justice services. Intake was slow, teams had difficulty coordinating across programs, and reporting took time. PivotPathways went live in early September 2026 and is now used across all Pivot services. The platform brings participant information together, simplifies intake and case management, and helps teams coordinate trauma-informed support across housing, education, employment and well-being programs. Pivot is still migrating historical records and completing the rollout.
The solution
Healthcare
Staff can use a centralized record to manage intake, enrollment and service documentation. Bringing this information together helps teams follow the support a young person receives across services and provides a basis for monitoring health services and outcomes.
Youth empowerment
PivotPathways brings program participation and service records together so staff can coordinate support and spend more time mentoring and working directly with young people. The records help teams follow each participant’s progress toward housing, education, employment and longer-term well-being.
